Nipple Problems 

 

It is a very common problem that the nipple is pulled into the breast tissue and collapsed. Another situation is that the nipple is larger than normal and protruding. Retracted nipples can trigger infections by accumulation of secretions in this area. Larger-than-normal nipples, on the other hand, can cause visual discomfort by being too obvious from clothes. These problems can be corrected by surgical methods.

What is nipple surgery?

Nipple aesthetics aim to create a more balanced appearance by adjusting the outward orientation/position, size, and contour of the nipple, as well as the shape and borders of the areola. Each patient's needs are different. The goal may not be solely "appearance." In some cases, we may also aim to preserve sensitivity, achieve functional harmony, and attain a natural look.

In what situations is nipple surgery necessary?

The following are generally the situations that necessitate an evaluation for nipple aesthetics:

  • Inverted nipple (low projection)

  • Uneven nipple size or a noticeable difference between the two sides.

  • Undesirable shape/size around the areola (areola width, irregular borders)

  • Asymmetry (nipple height, contour arrangement, centering issues)

With nipple aesthetics, we aim to improve a person's daily life comfort and self-confidence. There is no rule that every structural difference must be surgically corrected. The decision for intervention is made based on the individual's complaint, and the approach to be applied is planned according to the person's anatomical structure.

Inspection and Planning

The most critical step in nipple aesthetics is a detailed examination. We assess the current projection of the nipple, the structure of the areola, the degree of asymmetry, the elasticity of the skin tissue, and any previous interventions. We also evaluate the patient's expectations and realistic goals. ​

Criteria evaluated during the examination

  • Symmetry: Creating a balanced appearance between the two sides.

  • Natural contour: Maintaining a "as natural" look as possible.

  • Projection goal: To create extroversion if introversion exists.

  • Areola border: Defining the shape and border of the areola.

  • Tissue and sensitivity compatibility: The surgeon's approach is determined on an individual basis.

How are nipple surgeries performed?

Nipple surgeries can be performed alone or in combination with other breast surgeries. Nipple surgeries are treatments that can be performed under local anesthesia on an outpatient basis.

To correct inverted nipples, a 1-1.5 cm incision is made at the edge of the nipple; the tissues causing the nipple to retract are released, and permanent sutures are placed to prevent the nipple from retracting again.

It is important to remember that congenital milk duct collapse can lead to developmental abnormalities in the milk ducts and cause problems with breastfeeding.

In most cases, the cause of inverted breasts is the stiffness or shortening of the mammary duct and surrounding tissues. In this situation, sacrificing the mammary duct may be necessary to correct the problem. If the person plans to breastfeed, the surgery can be performed after the breastfeeding period ends.

Reducing the size of large nipples

Various methods have been described for reducing the size of wide or abnormally long nipples. Techniques that do not damage the milk ducts and that create an aesthetically pleasing appearance are preferred. The choice of technique is made based on the patient's examination findings.

Nipple reduction can be performed in conjunction with any breast aesthetic surgery, or it can be performed alone under local anesthesia.

The first few days:

Edema, mild tenderness, protective measures applied to the area, and wound care.

First 1-2 weeks:

Wound care should be provided, regular check-ups are necessary, and trauma should be avoided.

4-6 weeks

The dissolvable sutures disappear. Scar care can be provided if needed.

2-3 months:

The picture usually becomes clearer within a few weeks to months.

During the healing process, factors such as smoking, excessive movement/stretching, and failure to protect the affected area can affect the outcome. Therefore, individualized instructions should be followed.

Will there be scarring after nipple aesthetic surgery?

Depending on the technique used, scarring may occur. The goal is to leave scars as close to natural boundaries as possible and, if possible, in less noticeable areas. The appearance of the scar varies depending on skin type, healing quality, and aftercare.

When will things return to normal?

It depends on work/lifestyle. In general, daily activities can be resumed within a short period; however, it is beneficial to wait a while before engaging in strenuous sports or activities that may put pressure on the area.

Are nipple problems congenital?

It is usually congenital (primary) and may become apparent during puberty. There are also acquired (secondary) conditions; examination is absolutely necessary, especially if it is unilateral and there is a change.

Is breastfeeding possible after surgery?

This varies depending on the scope of the surgery. Sometimes, sacrificing breastfeeding function may be necessary to correct the nipple. The decision is made on a case-by-case basis.
