What is Filler?

 

Fillers are substances that are produced chemically or in a way that can replace the structural substances in our body obtained from bacteria. In general, they contain hayluronic acid, which is one of the basic building blocks of subcutaneous tissue. Hyaluronic acid is a substance that provides support throughout the body, especially bone, cartilage and subcutaneous tissue. 

What is the purpose of FILLERS?

Dermal fillers are most commonly used in cosmetic surgery to support facial features, restore lost volume, and soften contours/lines.

Corrects volume loss: Reduces age-related hollowness in areas such as the inner cheeks/midface, temples, and around the lips.

Contour shaping: Can create more defined/harmonious lines in areas such as the cheekbone line, jawline, and around the tip of the nose.

Smoothing fine wrinkles: Especially expression lines and superficial contours can be made smoother with a filling effect.

Eliminating sunken areas: It can be used supportively in some areas, such as under-eye hollows (in suitable patients, with the right product/technique).

Lip fullness and lip contouring: Increasing lip volume and defining lip boundaries.

Correcting facial asymmetries: Can be used to balance differences between the left and right sides or to correct pronounced depressions.

FILLERS USED IN COSMETIC SURGERY

In cosmetic surgery/aesthetic procedures, "fillers" are generally grouped into two main categories: short-to-medium-acting (the most common) and long-acting/biostimulant products. The most common types of fillers are:

Hyaluronic Acid (HA) Based Fillers

  • Cross-linked hyaluronic acid gels

  • It is commonly used for adding volume, contouring, lip augmentation, cheek/cheekbone enhancement, nasolabial folds, jawline correction, and treating certain scars/depressions.

  • Depending on their composition, the products are manufactured in different densities.

  • It is the most commonly used filler material.

Calcium Hydroxyapatite (CaHA)

  • It exists as microcrystals within a carrier gel.

  • Generally provides more pronounced volume support and long-lasting contour correction (e.g., in suitable patients such as those with mid-face/jawline issues).

Polylactic Acid (PLA) / Biostimulator Fillers

  • It targets collagen stimulation in multiple stages, rather than directly increasing volume.

  • It is generally used for tissue tightening over time and facial rejuvenation (e.g., cheeks/midface).

Polymethyl methacrylate (PMMA)

  • It contains implant-like microspheres that are considered to be more "near permanent".

  • Its use is more restricted and requires significant training/experience (incorrect application can be difficult to reverse).

WHICH AREAS ARE FILLERS APPLIED TO?

Lip Filler (Lip Augmentation and Contouring)

Lip fillers reduce volume loss in the lips, providing a fuller and more balanced appearance. They also help to define the lip contour, adjusting the corners of the lips and the proportions of the upper and lower lips.

  • It supports contour balance.

Cheek and Cheekbone Augmentation (Mid-Face Rejuvenation)

The cheek and cheekbone area is one of the areas that most affects the perception of youthfulness on the face. Filler treatments can help the mid-face appear more balanced and "lifted," thus making the facial contour more harmonious.

Jawline and contour fillers are preferred for shaping and proportioning the lower part of the face. The aim is to make the jawline more prominent and enhance the overall facial appearance. When the procedure is planned individually, the facial features appear more harmonious.

⚠️ SAFE FILLER APPLICATIONS

We prefer licensed fillers: Products approved (licensed) by the Ministry of Health and the FDA should be preferred.

We do not use counterfeit/substantial products . Products with unknown batch numbers, expiration dates, and storage conditions should be avoided.

We check the product batch and expiration date: Pre-injection verification is required.

The product is opened and used individually : Fillers are available in 1 or 2 ml pre-filled syringes.

The injection should be administered by a qualified physician: Filler injections are a medical procedure and should be performed by specialist physicians who have received adequate training in this area.

Product-area compatibility is important: Not every filler is suitable for every area; the correct indication must be chosen.

We are questioning the patient's history: Allergies, autoimmune diseases, active infections, and a history of herpes should be investigated.

We perform the procedure under sterile conditions: Sterilization and hygiene standards must be followed prior to application.

Blood thinner use is monitored: Medications and supplements should be communicated in advance to plan the procedure.

Seek prompt medical attention in case of alarming signs : Severe pain, discoloration, spreading of the skin, or impaired vision should be reported without delay.

How is a filler injection performed? (Application process)

Examination and Personalized Planning

Filler treatment requires personalized planning. The first step involves evaluating facial proportions, skin type, any asymmetries, and the individual's expectations.

We plan which area will be treated, and with what type and amount of filler. This allows us to aim for a more natural-looking and balanced result.

  • Facial proportions and needs analysis are performed.

  • The desired appearance and realistic goals are determined.

  • An appropriate type and quantity of fill material will be determined, and a regional plan will be created.

Pre-Application Preparation

Before the procedure, medical history and current medication use are thoroughly reviewed to minimize risks.

  • Medical history and medication/supplement use are evaluated (blood thinners, herbal supplements).

  • The procedure may be postponed/the plan updated if necessary (cold sore, breastfeeding).

  • The area is cleaned and marked.

  • A local anesthetic may be applied to the treatment area.

Filler Application

  • The filler material, in pre-filled syringes opened specifically for each individual, is applied at the appropriate depth and dosage.

  • The procedure takes 20-30 minutes.

  • Very little pain is felt during the procedure.

  • The person can resume their work and social life after the procedure.

  • Makeup can be done the next day.

  • Avoid extreme heat/sauna/hammam during the first 24–48 hours.

Some results from filler injections are noticeable immediately after the procedure; however, swelling and slight bruising can cause variations in appearance during the first few days. For most people, the true result usually becomes apparent within a few days as the swelling subsides.

  • A clearer picture usually emerges within a few days.

  • Results will typically be visible within 7-10 days, once the swelling has largely subsided.

  • In stable regions, it can last 12-18 months.

  • Gradual approaches are preferable for a “natural and balanced” outcome.

Risks and Complications in Dermal Filler Applications

Filler injections are safe for most people; however, rare situations requiring urgent evaluation may arise. Symptoms such as severe and increasing pain after the procedure, noticeable changes in skin color (such as pallor/bruising), rapid spread of the treated area, or, very rarely, impaired vision should be taken seriously. In such cases, it is important to contact the clinic/doctor who performed the procedure or the nearest healthcare facility immediately.

  • Severe and progressively worsening pain

  • Paleness, dark discoloration, or noticeable bruising in the treated area.

  • Rapid spread of swelling or hardness

  • Blurred vision/double vision/light-related changes

  • Shortness of breath, widespread rash, rapid swelling around the lips and eyes (suspected allergy)

Precautions for unwanted effects

Dermal fillers are medical procedures, therefore it is safer for them to be administered by individuals who have received extensive specialized training in this field. Every procedure carries risks, but the probability of risk decreases when the procedure is performed by knowledgeable and experienced professionals. Steps to reduce the probability of risk include:

  • The procedure should be performed by a physician familiar with vascular anatomy.

  • The correct technique, the correct layering, and a controlled dose should be targeted.

  • Appropriate patient selection and risk assessment should be performed.

  • An emergency plan should be in place and information should be provided regarding the post-operative situation.

  • If suspicious symptoms appear, report it without delay.

When does the filler show its effects?
