Who is a Good Candidate ?

Breast lift with implants (mastopexy-augmentation) is specifically designed for patients experiencing both breast sagging (ptosis) and significant volume loss.

Ideal candidates include those who meet the following criteria:

  • Your nipple-areola complex has dropped to or below the level of the inframammary crease (breast fold).

  • You notice a complete lack of fullness or a "hollowed-out" appearance in the upper pole (upper half) of your breasts, especially when wearing a bra.

  • You desire your breasts to be both youthfully elevated and significantly more voluminous than their current size.

In what situations might breast lift with implants not be a suitable procedure?

In cases such as uncontrolled diabetes, severe cardio-pulmonary disease, coagulation (bleeding) disorders, active infections, or any suspicion of breast malignancy, the necessity and safety of the surgery must be comprehensively re-evaluated.

Is breast augmentation with implants safe during breastfeeding?

No, it is essential to wait for hormonal levels to return to baseline after breastfeeding is completed. The surgical procedure can typically be performed approximately 6 months postpartum/post-weaning.

Is smoking a barrier to surgery?

Smoking is a habit that adversely affects all stages of breast augmentation and lift surgery, significantly increasing the risk of post-operative complications. Therefore, complete cessation of smoking prior to the procedure is highly recommended as the most ideal approach.

PRE-OPERATIVE ASSESSMENT

The combination of a breast lift and silicone prosthesis (mastopexy-augmentation) requires meticulous planning, as two distinct surgical procedures are executed simultaneously. To achieve a successful and long-lasting outcome, we comprehensively analyze the following parameters during our pre-operative clinical evaluation:

  • Breast Tissue and Volume Analysis: The volume of the existing breast tissue and the degree of hollowing in the upper pole (cleavage area) are evaluated. This analysis is critical for determining the precise volume of the silicone implant required during the lift.

  • Chest Wall and Anatomical Measurements: The width of your chest wall, the distance between the two breasts, and the breast base width are measured with millimetric precision. These measurements ensure the selection of an implant base diameter that is perfectly compatible with your body contours and does not protrude outward.

  • Nipple-Areola Complex and Degree of Sagging (Ptosis): The distance and orientation of the nipple-areola complex relative to the inframammary fold (breast crease) are measured to determine the stage of sagging (mild, moderate, advanced). The exact point to which the nipple will be repositioned during surgery is planned at this stage.

  • Tissue Coverage and Skin Quality: The skin thickness in the upper pole (pinch test) and the elasticity/stretch mark condition of the skin are examined. This data is our most important guide when deciding whether the implant will be placed submuscularly (under the muscle) or via the dual-plane approach. It also provides insights regarding potential future tissue laxity.

  • Asymmetry and Chest Wall Evaluation: Differences between the two breasts in terms of size, shape, or position (asymmetry), as well as potential bone asymmetries in the chest wall, are identified.

Following the clinical evaluation and surgical planning, the general medical assessment, breast imaging, and anesthesia consultation are completed.

In breast lift procedures where there is sufficient breast tissue, the two surgeries are similar. However, breast reduction focuses on reducing volume, while breast lift focuses on reshaping the existing volume.

SURGICAL TECHNIQUE

In breast lift and augmentation surgery (mastopexy-augmentation), personalized planning is crucial. For patients requiring this surgery, the desired result cannot be achieved with silicone implants alone or with lift techniques alone; two different surgical procedures must be performed simultaneously.

  • Implant Geometry and Profile Selection: We generally prefer round implants because we aim for maximum fullness and a defined projection in the upper pole (décolleté area).

  • Prosthesis Placement Plan (Anatomical Pocket): In patients with insufficient tissue coverage, a dual-plane (semi-submuscular) approach is preferred to utilize the strength of the pectoral muscle while preserving natural movement, ensuring the edges of the prosthesis are not visible and providing a natural transition. If there is sufficient tissue coverage, a subfascial or subglandular plane is preferred.

  • Incision Modification According to the Degree of Sagging: The incision line to remove excess skin is determined according to the stage of breast ptosis (sagging). For mild and moderate sagging, a lollipop (vertical) incision extending downwards from around the nipple is sufficient; however, in cases of severe sagging and significant excess skin, an inverted T (anchor/wise pattern) incision following the inframammary fold is preferred.

  • Implant Surface Technology: To minimize the risk of potential foreign body reactions such as capsule contracture and to maximize tissue compatibility, we use reliable, FDA-approved, next-generation silicone prostheses with biocompatible surface technology in our operations.

  • Implant size: The goal is not to create very large volumes, but to bring the breast to a balanced and body-compatible size.

Surgical process

Breast lift (mastopexy) surgery is performed under general anesthesia, takes approximately 2-3 hours, and requires a one-day hospital stay for observation. A medical bra is applied after the surgery. Drains are generally removed upon discharge.

RECOVERY PERIOD

  • First Few Days: You may feel a little tired due to the effects of the anesthesia. Mild aches and pains are normal. Gentle movement around the house will help with recovery.

 

  • Week 2: Swelling and pain in the surgical area decrease, movement becomes easier, and it becomes possible to return to work and daily activities.

  • 6-8 weeks: The breast begins to shape, stitches heal, swelling subsides, and you can return to sports that require arm movement.

POST OPERATIVE CARE

  • Medical bra : Wearing a medical bra during the first month helps reduce swelling and aids healing.

  • Medication: In the first few days, simple painkillers and prophylactic antibiotics are taken. After the dressings are completed, creams are used to promote scar maturation.

  • Sleeping position: Sleeping with your torso slightly elevated using two pillows is helpful for controlling swelling.

  • Check-ups: Regular check-ups are performed, more frequently in the first few weeks, and dressings are changed during these check-ups. The stitches dissolve on their own.

  • Nutrition: Drinking plenty of fluids and consuming a diet rich in protein is important. Avoiding smoking and alcohol is recommended.

RISK and COMPLICATION

As with any surgical procedure, breast lift (mastopexy) surgery carries certain risks and potential complications. Risks and undesirable outcomes vary depending on factors such as the individual's breast structure, general health status, smoking habits, and the specific technique used.

General Surgical Risks

Bleeding (hematoma), seroma (fluid accumulation), infection, delayed wound healing, and anesthesia-related complications may occur.

Risks Specific to Breast Lift and Augmentation

  • Nipple circulation problems, asymmetry, prominent scarring, changes in nipple and skin sensation, and pigment changes may occur.

  • Effects on Breastfeeding: Since milk ducts can be affected, breastfeeding capacity may decrease; although breastfeeding remains possible for some patients, it cannot be guaranteed.

  • Incompatibility Between Breast Tissue and Implant: A breast lift combined with augmentation is a blend of two different surgeries. The breast tissue and the implant may undergo different shaping processes, which can lead to minor asymmetries.

Implant-Related Risks

Implant-related issues such as capsule contracture, implant rupture, deformity, and BIA-ALCL may develop in the late period.

Does performing both a facelift and inserting silicone implants at the same time complicate the healing process?

No, the recovery process is similar to other breast surgeries, and it's generally possible to return to daily life within 7-10 days.

Can I breastfeed after breast lift surgery with implants? Will the milk ducts be damaged?

This combined surgery does not interfere with future breastfeeding function. Using modern surgical techniques, the nipple and areola complex are moved upwards while preserving the milk ducts and nerve connections, but breastfeeding function cannot be guaranteed.

How long does it take for the breast to take its final shape and regain its natural softness after surgery?

The softening of the tissues, the precise placement of the implant into its anatomical pocket, and the breast's natural teardrop shape take approximately six months. The complete maturation and fading of scars takes about a year.

Is there a risk of the silicone implant slipping or shifting in the early post-operative period?

During the early recovery period (first 4-6 weeks), the surgical pocket where the implant was placed has not yet fully closed and stabilized biologically. To prevent the implant from moving upwards or shifting to the side during this time, it is vital to wear the special medical support bra prescribed by your doctor continuously, as instructed.

Is this combined surgery suitable for those planning to become pregnant again in the future?

Medically, there is no problem with having this surgery before pregnancy, and the operation does not prevent pregnancy. However, pregnancy and breastfeeding cause the breast skin and tissue to stretch again due to hormonal changes.
